Social prescribing is a non-medical, holistic approach that recognises that a person’s health and wellbeing are influenced by a wide range of social, environmental and economic factors. Rather than focusing solely on a medical condition or disability, social prescribing views each person as an individual and supports their overall wellbeing.
Through social prescribing, GPs, nurses and other primary care professionals are able to refer patients to a designated Social Prescriber within their health network. This allows patients to access face-to-face conversations where they can discuss opportunities to improve their emotional and mental health and wellbeing. Individuals with social, emotional or practical needs are supported and empowered to identify and develop personal solutions that are meaningful to them, often by accessing services within their local community and the third sector.
Afan Cluster Social Prescribers provide both emotional and practical support to patients, particularly those who may be experiencing difficulties related to their emotional or mental health and wellbeing. When a patient aged 18 or over attends their GP practice and a healthcare professional identifies that they may be struggling with emotional distress or mental health concerns, they may be offered an appointment with a Social Prescriber. These appointments provide an opportunity for patients to discuss their concerns, work through challenges, and develop coping strategies to help manage their wellbeing in a more holistic way.
The Social Prescribing team works across all GP practices within the Afan Cluster, sharing services, expertise and resources across the cluster area. This collaborative approach supports effective partnership working between GP practices, health and social care services, and the voluntary sector.
This service is available to all patients aged 18 and over who are registered with a GP practice within the Afan Cluster.
